SUMMARY DESCRIPTION OF WORK
This Care Team Program Coordinator supports the Community-Based Services Program Director in overseeing Care Team Specialists across Pottawattamie County and the Southwest Iowa region. This position ensures high-quality care coordination for Heartland Family Service clients by supervising staff, implementing best practices, and fostering collaboration with internal and external providers. This individual agrees to be highly organized, detail-oriented, and able to manage competing priorities while promoting a trauma-informed workplace. Regular travel to regional offices and CCBHC hubs is required.
Compensation: between $49,200 and $62,243 per year (salary is determined by total years of experience)
Work Schedule: 40 hours per week
